Instructions for Filling Out Forms for the First Visit

There are three forms to be completed and/or read at the beginning of the first visit. If you want to do this beforehand to save time at your appointment they can be downloaded from this web site.

1. The Permission and Authorization Form needs to be read and signed prior to being seen at the office. Be sure to bring it with you if you decide to complete it beforehand.

2. The Personal Health Story needs to be read.

3. The Nutrition Response Testing New Patient Orientation Form needs to be read. This should be read carefully because it answers a lot of questions about what we do and will save you a lot of time.
